Update Personal Info

Address Change

If your address has changed within the last 90 days you will need to notify the CSWMFT Board. 

You will be asked to list both a Mailing Address, which we can use to reach you if needed, and a Public Address.  The Public Address is public record, and can be accessed upon request, however your full Public Address will NOT appear on the Board's website; only the City, State, County, and Zip Code will display in the online license verification.

Instructions:

  1. Visit elicense.ohio.gov
  2. Click Login / Create Your Account
    • Log in as an Existing User with your User Id (your email address) and the unique password you created when you registered.  
  3. Then, go to your license “tile” (it looks like a mini certificate) and click on “Options.”
    • From the drop-down menu under the “Options” button you can choose to:
      • Renew
      • Reinstate
      • Duplicate/Replacement Wall Certificate
      • Change Address
      • Change Name (licensees with multiple licenses should indicate in the comments which license certificates to replace) 
        • ​Only one name change fee must be paid even if certificates for two or more licenses are requested, e.g. MFT and LPC wall certificates.  ​
      • Send Verification
      • Special Request
      • Submit additional Documentation 

Name Change

THE NAME LISTED IN THE BOARD'S DATABASE IS THE ONLY NAME YOU ARE LEGALLY ABLE TO PRACTICE UNDER. If your name has changed within the last 90 days you will need to notify the OCSWMFT Board of your NAME CHANGE during the renewal process.

Instructions:

Email

The Board’s primary way to reach licensees is by email. It is important you have an email address and that you check it regularly (please see 4757-5-11(B)). It has come to the Board’s attention that many of the emails sent by the Board, CE Broker and Ohio’s e-license system are seen as spam and a lot of these emails end up in junk mail folders.

To ensure all emails reach you please add the following emails to your contacts/safe senders lists:

  • elicense-noreply@das.ohio.gov to receive renewal notice emails
  • notifications@cebroker.com  to receive notice of the CE Audit
  • CSWMFT.Info@cswb.ohio.gov to receive the Board’s mass emails to licensees and other important information

How to Update your Email Address

  1. Visit elicense.ohio.gov
  2.  From your dashboard: Click the little triangle next to your name in the upper right-hand corner, and click on Manage Profile.

  1.  In the personal Information section you can update your email address. Your new email address is now the User ID  you will use the next time you sign into the Ohio e-license system.
