Renewal Requirements:

Cost of Renewal: Licensees have until midnight of their renewal date to renew their license.
SWA: $40 (late fee: $20)
LSW, PC & MFT: $60 (late fee $40)
PCC, LISW & IMFT: $75 (late fee $ 40)

Renewal CEU Requirements:
All licensees are required to take thirty clock hours of continuing education for each two year renewal period. Three of the thirty must be in ethics.

Supervising Professional Counselors are required to have six hours of supervision CEUs that meet the requirements of paragraph (F)(1)(a) of rule 4757-17-01 of the Administrative Code.

Supervising Independent Social Workers renewing after 1/1/2009 are required to have three hours of supervision CEUs that meet the requirements of paragraph (D)(3) of rule 4757-23-01 of the Administrative Code.

All CEUs taken within the State of Ohio must be Pre-Approved by the CSWMFT Board. Programs approved by the CSWMFT Board will list their approval number. Approval numbers from this Board begin with the letter R or the letter M and will also contain a letter that represents your licensure.

Out of state programs, online or home study courses that have not been pre-approved, can be submitted for post approval. After taking the program you can receive approval by completing a post program approval form.

Of your 30 CEU hours social workers and MFTs may do all 30 as DISTANCE LEARNING (i.e. home study, teleconference, and internet based programs). Counselors may only do 15 of 30 as distance learning. There are many distance learning programs that are approved by this Board, however if they are not approved you can fill out a post program approval form.

Renewal Notice Mailings:
Renewal applications are mailed in quarterly increments and mailed in:
November for January through March renewal dates;
February for April through June renewal dates;
May for July through September renewal dates;
August for October through December renewal dates.
They are mailed prior to renewal and include your online User ID and password. If you do not receive an application you should contact the board's office prior to your renewal date to request a UserID & Password.

Renewal Online:
All renewal notices contain the licensee's user ID and password. If you have misplaced or never received your renewal notice, you can contact the Board and we can give you your user ID and password over the phone or through email.

When you renew online your license will be renewed the next business day, but it will take up to three weeks for you to receive your wallet cards.

Requesting your Renewal Notice:
If you did not receive your renewal notice through the mail you may call or email the CSWMFT Board and request a UserID and Password for online renewal. Paper applications require a written request, email is accepted. It may take up to 3 weeks to reach you.

Felony/1st degree Misdemeanor:
If you have been convicted of a felony or first degree misdemeanor since your last renewal or since licensure you are required under law to report all misdemeanor and/or felony convictions (other than minor traffic violations). Failure to report any police record is falsification of an application and is grounds for denial of an application for renewal.

CEU Attendance certificates:
DO NOT send in copies of your attendance certificates with your renewal.

KEEP PROOF OF YOUR CEUs FOR TWO YEARS AFTER EACH RENEWAL. If you are audited in the, you will be asked to show proof of your CEUs from the most recent renewal submitted.

Verification/proof of renewal:
Please note: once your renewal application has been received at the board office or by its agent, you may continue to practice unless otherwise notified by the board. The intent to comply with the law is in filing the application for license renewal. If proof of receipt is required by your employer you will need to submit your application via certified mail. Confirmation of receipt is not sent out by the board.

You, not the Board, are responsible to provide proof of renewal to your employer.

Follow the steps below for proof of your renewed license:

Click on license verification fill in only two areas Division and Name (Last, First)

Beside Division set the drop down menu on Counselor, Social Worker, and Marriage & Family Therapist Board

Scroll down to -or- Name (Last, First) in the first box type your last name in the next box type your first name click the search button. (No hyphens, no middle initials) IMPORTANT! Fill in no other boxes!
Once the page re-loads click on your NAME (bottom of page). The page that comes up next will be the page you print for license verification.

New Mailing address:
Renewal Notices are mailed to the address you have listed with the Board so be sure your address is correct with the Board.
(K) Notice of name and address change
All licensees or registrants are required to notify the board of any changes of name or information to ensure that the board has their current name and mailing address. Failure to do so may result in disciplinary action by the board.


If you did not receive your renewal notice through the mail you may call the CSWMFT Board and request an online UserID and Password. Paper renewal forms are only sent upon written request (emails accepted). It may take up to 3 weeks to reach you. You may also request your user ID and password to renew online and to update your address, if incorrect.

You may e-mail, fax, mail or call the Board with your new mailing address. The Board also offers an address change form on our web site.

Proper form of payment:
When renewing through the mail we cannot accept personal checks. We will accept business checks, certified checks from a bank and money orders.

When renewing your license online we only accept visa or master card. ( Check Card, Debit cards, or Prepaid-Re-Chargeable cards with the VISA or MASTER CARD logo are acceptable. NO GIFT CARDS)

Name change:
Please include the NAME CHANGE form, with your renewal notice. The form is available on our web site.

Fill in the top two boxes and be sure to include documentation (your marriage license or drivers license in your new name or a court record).

Notarized application:
All paper renewal applications must be notarized. If living outside the state of Ohio the notary can cross out 'State of Ohio' and enter the state where you presently live.

Early Renewal:
Many licensees will receive their renewal notices three to six months in advance. If you renew your license early it is important to remember you cannot begin taking CEUs for your next renewal until your renewal date has passed.

Late Renewal:
If you have not completed your CEUs, do not renew your license until all CEUs have been completed.

If you are not practicing within the state of Ohio, the CSWMFT Board does allow for late renewal. However, you cannot skip a renewal. If you have not renewed your license by your renewal date you are not able practice or refer to yourself as licensed.

If the license has lapsed for less than two years, and you have submitted a renewal application and fee, the license will be renewed to the next renewal date.

Example:
License renewal date is: June 20, 2008
Renewal application & fee submitted: March 8, 2009
Next renewal date will still be: June 20, 2010


If you allow your license to remain in a lapsed status beyond your next renewal date your license will expire and you will have to re-test and re-apply for licensure under the current Laws and Rules.

Waivers (Extensions): The board no longer provides extensions, but can provide a waiver for extra time to complete your CEUs.
Waivers for the completion of continuing education are granted for extenuating circumstances. A written request should be submitted prior to your renewal date. The request must include an explanation for not completing the requirements, and the estimated amount of time needed to complete the requirements. For medical reasons please submit a physician's statement.

You are also able to receive a waiver for course cancellations. Notices of course cancellations should be submitted with your written request.

Waivers ARE NOT granted if you are not practicing within the state of Ohio.

License Verification Status Meanings:
You will see the following statuses on license verification:
1. "Active" - license is active and not due for renewal
2. "Active in Renewal" - Licensee has been sent their renewal information- licensee is active and able to practice according to expiration date
3. "Active in Renewal - PAID" - licensee has renewed their license and is active and able to practice through the expiration date
4. "Failed to Renew - PAID" - Licensee has renewed their license late- able to practice through the expiration date- active and able to practice
5. "Failed to Renew" - Licensee has not renewed their license – not able to practice or to refer to her/himself as licensed - Not Active
6. "Expired" - license was not renewed within two years of expiration date and licensee will have to re-apply for licensure unless they hold a higher license
7. "Upgrade" - Previously active license that has been upgraded to a higher license

Post Program CEU Approval
Licensees can receive program approval for the below types of CEUs not approved by the Board. Find forms under the CE Forms link: CE Forms page
1. Out of state programs
2. Online programs
3. Home study courses
4. College coursework taken in a related discipline, other than that of your license. Coursework taken within the department of your license is automatically approved
5. Publications, Articles, Books and Teaching

CEUs taken within the State of Ohio must be Pre-Approved by the CSWMFT Board and are not eligible for post program approval. An exception to this rule: Marriage & Family Therapist are allowed post program approval for programs, coursework in related disciplines as well as programs approved for counselors and social workers. This is due a limited amount of approved programs for MFT’s.

Post Program approval forms are to be submitted within ninety (90) days of attending a program. To obtain post-program approval, the licensee shall provide the following: ref: 4757-9-05
1. Documentation that the program presenter(s) met the requirements of paragraph (A)(2)(a) of this rule;
2. A copy of the program brochure and other documentation, if necessary, demonstrate that the program met the requirements of paragraphs (A)(2)(b) to (A)(2)(f) of this rule;
3. A certificate of completion, transcripts, grade card, or signed statement from the presenter to demonstrate the licensee’s attendance at the program or course.

Expect three weeks for your post program approval form to be evaluated. You will then be sent an approval letter telling you how many CEU’s you may count towards your renewal.

Do not renew your license until you have completed all CEU’s and have gotten your notice of action letting you know how many CEUs you may count towards your renewal.
