If you cannot download the forms below, or you do not have a computer, you can print off forms from a computer at your local library. You may also mail, call 614-466-0912 or e-mail your request for forms. Be sure to include the following information.
Name & Address & City/State/Zip & if you have one your License Number.

The Adobe Reader is required to view these forms. Versions for Windows, DOS, Macintosh or Unix are available FREE. Forms are Active Adobe Forms meaning you can click on the text boxes and type in your information and then print for mailing.

Renewal Forms

Renewal Applications must be requested from the board office. You can renew online using the UserID & password printed on your renewal notice or by emailing or calling the office for your UserID and Password (no paper is required). You can renew online at Online Renewal with your UserID and Password. Please call 614-466-0912 or e-mail your request.
